Conditions of use and translate the QUALIOST®:

1. Use and Translation Agreement:

User is required to complete and sign the "Use and Translation Agreement for the QUALIOST®" in which specific conditions of use and translate required by the copyright owner are detailed.

Important note: the QUALIOST® is a self-administered module consisting of 23 questions which complements the SF-36. The generic items (SF-36) along with the disease-specific items forms the whole QUALIOST®. The disease-specific items cannot be used as an independent instrument. Please refer to the user agreement.

2. User's fees:

carre6 Distribution fees is requested according to the study design and context of use of the questionnaire:

  1. In not-funded academic research (1) and individual clinical practice: free of charge
  2. In funded academic research (2): 300 €* per study + an additional 50 €* per language version requested.
  3. In commercial studies (3) involving "for-profit" organizations: 600 €* per study + an additional 250 €* per language version requested.


* excluding VAT

(1) Not funded academic research: if the project is not explicitly funded, but funding comes from overall departmental funds or from the University or individual funds then fees are waived.

(2) Funded academic research: projects receiving funding from commerce, government, EU should anticipate paying the corresponding fees.

(3) Commercial studies (industry, CRO, any for-profit companies).
