1. Academic and Clinical License:

The FIQ is free for academic and clinical use.

To develop FIQ translations, please see below conditions to translate.

2. Industry and Commercial License:

The use of the FIQ for industry sponsored research and other commercial uses is licensed by the MAPI Research Trust on behalf of the copyright holders. It is subject to specific conditions including a user fee and translation fee.
Details are provided below :

2.1.  User Agreement:

User is required to complete and sign a User Agreement in which specific conditions required by the copyright holders are detailed.

2.2.  Access fees:

carre6 Authors' royalty fee:

1. In commercial clinical studies: 1 000 €* per protocol/application plus an additional charge of 500 €* per existing language-version requested.
2. In commercial epidemiologic / observational / marketing studies: 1 000 €* per protocol/application plus an additional charge of 500 €* per language-version requested + the following administrative fee:
- Up to 1000 administrations/ reproductions: 2 €* per administration/ reproduction;
- Between 1001 and 2500 administrations/ reproductions: 1.80 €* per administration/ reproduction;
- Between 2501 and 5000 administrations/ reproductions: 1.60 €* per administration/ reproduction.
- From 5001 administrations/ reproductions: 1.40 €* per administration/ reproduction.

Other specific conditions requested by the Author are detailed in the User Agreement.

carre6 Distribution fees:

For industry sponsored research and other commercial uses: 600 €* per study + 250 €* per language-version requested.

These fees are payable to MAPI Research Trust.

Conditions to translate:

The FIQ has already been translated into various languages. Distribution of the FIQ translations is processed as the orignal version. carre6 Performing new translations
To be granted permission to translate the FIQ, please complete the FIQ Translation Agreement.

The Translation Agreement shall complement the User Agreement. All FIQ translations must be conducted in close collaboration with MAPI Institute in Lyon, France and with the developers who jointly developed translation guidelines to fit the FIQ specificities. Developers of the FIQ owns the copyright on any new translation (refer to the User and Translation Agreements for more details about conditions of use).

carre6 Commercial and/or International studies
In order to undertake the linguistic validation of the FIQ questionnaire, funds are needed to support the whole translation process. carre6 Academic translations / Not funded research and Evaluation studies
If funds are not available, FIQ translation guidelines will be provided to allow academics and clinicians to use the FIQ in their institution.

Review copy / Original version:

The original 1991 FIQ was modified in 1997, when 1 item (climb stairs) was added to the physical function subscale, hashmarks were added to the visual analogue scales to make scoring easier, and the words "including housework" were added to the 2 work-related items so that housework was recognized as a legitimate type of work. To avoid any confusion between both versions, each version has been numbered as follows:

carre6 FIQ 1991 will be version 1.0.
carre6 FIQ 1997 revised will be version 2.0.
For any new study, use of the FIQ 2.0 shall be required.
