Conditions of use:

1. User Agreement:

User is required to complete and sign a User Agreement in which specific conditions requested by the Author are detailed.

There are two types of user-agreement, one specific to the use of the RLS-QOL in commercial studies, and one specific to the use of the RLS-QOL in clinical practice and not funded academic research.

2. Access Fees:

The use of the RLS-QOL in any academic research study is free from any Author’s royalty fees.

The use of the RLS-QOL by “for profit” organizations is subject to author’s royalty fees:
• Less than 100 administrations/reproductions =  105 €* per study, plus 105 €* per translated version (excluding US English original version)
• 101 to 1,000 administrations/reproductions = 1,050 €* per study, plus 210 €* per translated version (excluding US English original version)
• Greater than 1,001 administrations/reproductions = 2,100 €* per study, plus 210 €* per translated version (excluding US English original version)

carre6 Distribution fees are requested according to the study design and context of use of the questionnaire:

1. In not-funded academic research (1) and individual clinical practice: free of charge

2. In funded academic research (2): 300 €* per study + an additional 50 €* per language version requested.

3. In commercial studies (3) involving "for-profit" organizations: 600 €* per study + an additional 250 €* per language version requested.

(1) Not funded academic research: if the project is not explicitly funded, but funding comes from overall departmental funds or from the University or individual funds then fees are waived.

(2) Funded academic research: projects receiving funding from commerce, government, EU should anticipate paying the corresponding fees.

(3) Commercial studies (industry, CRO, any for-profit companies).
