The aim of this bi-modular instrument is to measure treatment related impact on subjects of diabetes medication and diabetes devices.

Conditions of use:

1. User Agreement:

User is required to complete and sign a User Agreement in which specific conditions requested by the Authors are detailed.

2. Access fees:

carre6 Authors' royalty fees:

- The use of these instruments is free from any Novo Nordisk’s royalty fee for all non commercial and academic research and clinical purposes (category A and B of Users).

- The use of these instruments in commercial studies (category C of Users) will be subject to Novo Nordisk’s royalty fees (payable to MAPI Research Trust on behalf of Novo Nordisk) of an amount of :
• 500 Euros per module and per protocol/application plus an additional 500 Euros per module and per available language version (original version and translated versions)

Other specific conditions requested by the Author are detailed in the User Agreement.

carre6 Distribution fees are requested to the User. The amount charged depends on (1) the context of use and (2) the User’s profile.

Three categories of Users have been identified:

Category A.    Students, physicians, clinical practice, not-funded academic research i.e. if the project is not explicitly funded, but funding comes from overall departmental funds or from the University or individual funds; then the access to the instruments is FREE.

Category B.    Funded academic research: projects receiving funding from commerce, government, EU, should anticipate paying the corresponding fees:
    - License fee per study: 300 Euros
    - + an additional Module fee of 50 Euros for the use of both the TRIM-D and the TRIM-D Device.
    - + an additional fee of 50 Euros per module and per available language version provided.

Category C.    Commercial studies (industry, CRO, any for-profit companies):
    - License fee per study: 600 Euros
    - Additional Module fee of 250 Euros for the use of both the TRIM-D and the TRIM-D Device.
    - + an additional fee of 250 Euros per module and per available language version provided.
