Objective:
The FIQR is a revised version of the FIQ and aims at measuring fibromyalgia patient status, progress and outcomes.

Conditions of use:

1. User is required to complete and sign a User Agreement in which specific conditions required by the Authors are detailed.

2. Access fees:

carre6 Authors' royalty fee:

The FIQR is free of royalty fees for academic and clinical use.

For industry sponsored research and other commercial uses: 1,000 €* per protocol/application + 500 €* per language-version requested.

Other specific conditions requested by the Authors are detailed in the User Agreement.

carre6 Distribution fees:

Distribution Fees are requested according to the study design and context of use of the questionnaire:

- In not-funded academic research (1) and individual clinical practice: free of charge
- In funded academic research (2): 300 €* per study + 50 €* per language-version  requested
- In commercial studies (3) involving "for-profit" organizations: 600 €* per study + 250 €* per language version requested


* excluding VAT

(1) Not funded academic research: if the project is not explicitly funded, but funding comes from overall departmental funds or from the University or individual funds then fees are waived.

(2) Funded academic research: projects receiving funding from commerce, government, EU should anticipate paying the corresponding fees.

(3) Commercial studies (industry, CRO, any for-profit companies).

For the production of new translations to be used in commercial studies, each translation shall be coordinated by MAPI Institute.

carre6 Commercial and/or International studies
In order to undertake the linguistic validation of the FIQR questionnaire, funds are needed to support the whole translation process. carre6 Academic translations / Not funded research and Evaluation studies
If funds are not available, FIQR translation guidelines will be provided to allow academics and clinicians to use the FIQR in their institution. The user will be required to sign a Translation Agreement which is available upon request from Mapi Research Trust.
